- Candidates must note Option 1 pay: Rs. 1,00,000 consolidated + Rs. 20,000 emergency visiting charges + Rs. 1,200/hr beyond 16 hrs/week.
- Candidates must note Option 2 pay: Rs. 1,00,000 consolidated + fixed % of CGHS rates on a case-to-case basis.
- Candidates must report to the Medical Superintendent's office, ESIC Hospital, Sec-3, Plot No. 41, IMT Manesar, Gurugram (Haryana), on 12/10/2026 from 09:00 AM onwards, and must note that no one will be entertained after 11:00 AM.
- SC/ST/PWD/Female candidates and Ex-Servicemen must pay nil, while all other candidates must pay Rs. 500/-, and candidates must pay only by Demand Draft in favour of "ESIC Fund Account No. 2", drawn on a scheduled bank and payable at Manesar, and must submit the DD along with the application form at document verification, knowing that fees are non-refundable and other payment modes are not accepted.
