- At Dr. Rustom Narsi Cooper Hospital, D Wing, First Floor, Cashier Department (Room 65), visit from 27.01.2026 to 04.02.2026 (11:00 AM–03:00 PM), excluding Saturdays, Sundays, and public holidays. Attach receipt photocopy and submit the filled form with documents to the Establishment Department, D Wing, First Floor, by 04:00 PM between 27.01.2026 and 04.02.2026.
